Third sector organisations to share in newly opened £1 million Fund in 2019 to support people with long term conditions. Over 150,000 lives improved over ten years
The Health and Social Care Alliance Scotland (the ALLIANCE) is delighted to announce that the Self Management Fund: Working Together to Strengthen Integration is now open for applications.
Since its launch in 2009, the Self Management Fund for Scotland has provided over £18 million in grants, supporting over 150,000 people across Scotland living with long term conditions and their unpaid carers.
The Fund will distribute £1 million to third sector projects based which build capacity of people with long term conditions and their unpaid carers to take the lead in their health and wellbeing. Projects will support the delivery of health and social care integration across Scotland and the delivery of the national health and wellbeing outcomes.
Self management puts people in the driving seat of their life, working in partnership with those around them to live well with long term conditions and as unpaid carers. Self management projects to date have covered a range of activities and areas of interest from self management courses to drama performances, peer support and everything in between.
The deadline for applications is Sunday 30th June, projects will begin in October 2019 for 12 to 18 months.
