The services provider will also provide management information on service utilisation and maintain the CCG EOL care register. Outcomes and payment metrics: The key outcomes that the services will be measured against are: a) an increase in the number of patient dying in usual place of residence; and b) fewer emergency hospital admissions. Payments to the investor will be determined by performance against these outcomes.
The investment needed is estimated to be the region of GBP 1.1 m per year for the [three year] life of the services contract. In addition funding, to support delivery of outcomes, will be made available by the CCG and through Big Lottery Fund Funding. Scope of the services: The coordination hub (Hub) will act as a single point of access for coordinating services and the urgent response team. It will maintain a CCG wide EOL care register
